Background The birth plan is a tool that allows the self-learning and thoughtful analysis of the women during the birthing process, facilitating their making of decisions and participation, in agreement with the bioethical principles of autonomy and no malfeasance. Goal: To understand the perception and satisfaction of women who presented a birth plan. Methodology: Qualitative, descriptive, observational, retrospective and cross-sectional study. The population of the study was composed of 21 women who presented a birth plan regulated in a Hospital ever (...) since the recording of the presentation of the birth plan was started until 2015. Place: Cartagena. Techniques/methods: Semistructured interview. Results-Discussion: Most women considered the plan as a useful self-learning tool on the meaning of the birthing process, as it favored the communication between the expectant mothers/health professionals. Our informants sought information related to the birthing process from informal sources; most perceived a level of respect of their wishes during the process of giving birth. The use of the birth plan did not imply the perception of loss of the degree of control during the birth, although attention to the written document could be improved. Conclusions The use of the birth plan is a very satisfactory and useful experience for their involvement in their self-care, although in some cases, the births did not proceed according to their plans. Nevertheless, the prevalence of the principle of Non-Malfeasance over Autonomy was evidenced. (shrink)

Ethical, Religious and Legal Arguments in the Current Debate over Euthanasia in Spain In the last ten years, there have been several cases in Spain (Ramón Sampedro, Leganés, Jorge León that have led to an intense social debate on euthanasia. The recent case of Inmaculada Echevarría, a woman suffering from a serious disease that kept her immobilized in bed, has revived the debate on euthanasia in Spain. On 18 October 2006 she held a press conference and publicly asked to (...) be disconnected from the ventilator that kept her alive. After a long ethical, religious, legal, and social debate, the patient was disconnected on 14 March 2007 after being adequately sedated. As a consequence, the patient died. In our paper we defend the need for a radical and intercultural democracy and present the main ethical, religious and legal arguments on euthanasia that are being posed in Spain and in Europe as a debate that should help to build a radical and intercultural democracy at a European level. This paper provides an interpretation of Sein und Zeit §34, in the context of the existential analytic of Dasein. Dealing with the Rede – Sprache distinction, it tries to show how in Heidegger's conception, at least at the time of writing his main work, language is an existentially founded phenomenon that takes root in the prelinguistic opening of a space of meaning. In the light of this ontological structure of meaning and language it is possible to make sense of the (...) analyzed text and to do justice to the systematic place of language in the broader philosophical project of Sein und Zeit. En la Universidad Médica “Carlos J. Finlay”, se realiza una intervención educativa con el objetivo de valorar la concepción y aplicación de un programa para un curso de preparación de metodólogos en correspondencia con sus funciones en condiciones de universalización de las Ciencias Médicas de Camagüey, en el periodo de septiembre de 2005 a junio de 2007. Se emplean diferentes métodos investigativos: los teóricos, así como la técnica de discusión grupal, permitió diseñar el programa del curso para la preparación de (...) metodólogos en condiciones de universalización y los empíricos, la obtención de algunos datos de interés acerca de los metodólogos que reciben el curso y sus valoraciones en relación con el mismo. Se constata que la preparación pedagógica y experiencia en el desempeño es insuficiente para asumir el rol de metodólogo y valoran de muy importantes los contenidos desarrollados, los métodos y formas organizativas concebidas, por lo que se concluye que el programa del curso cumple con las expectativas del metodólogo al estar diseñado teniendo en cuenta las funciones que estos desempeñan y sus necesidades sentidas A study was carried out about the intervention in education developed to increase teacher's methodological preparation under their new functions due to the fresh conditions of learning within the community applied to medicine training and education in Camagüey. It was carried out in the period of September 2005 to June 2007. Different research methods were used: the theoretical ones and the technique for group discussion help to design the course syllabus and the assessment for those teachers that received the course. It was verified that the methodological preparation and performance experience are insufficient to play their role; but they value as very important the contents, the methods and the organization of the course. The authors conclude that the course meets the expectations of training, because it was designed keeping in mind the teachers' functions and their necessities. (shrink)

Se realizó un estudio descriptivo para evaluar la influencia de la rehabilitación protésica en la calidad de vida de los pacientes desdentados totales tratados en las clínicas estomatológicas docentes "Ismael Clark Mascaró" y "La Vigía" en Camagüey, desde septiembre del 2009 a septiembre del 2013. El universo estuvo comprendido por 254 pacientes desdentados totales rehabilitados y una muestra de 43, a través de un muestreo probabilístico aleatorio. Se obtuvo un predominio del grupo de 50-69 años y el sexo femenino. Al (...) determinar los problemas mecánicos se apreció que los flancos sobreextendidos, seguido de las inserciones frénicas sin liberar y la presencia de puntos prematuros de contacto, fueron las principales dificultades encontradas en los pacientes portadores de prótesis totales. La rehabilitación influyó en la calidad de vida de estos pacientes. It was carried out a descriptive study with the objective to evaluate the influence of prosthetic rehabilitation in total toothless patients life quality treated in the teaching stomatological clinics ¨Ismael Clark Mascaró" and "La Vigía" in Camagüey, from September 2009 to September 2013, with a universe of study composed by 254 total toothless patients who were rehabilitated. The sample was of 43, through a random probabilistic sampling. There was a predominance of the female group aged from 50-69 years old, followed by the female group aged 70. While determining the mechanical problems it was appreciated that the flanks overextended, followed of phrenic insertions without being sustained and the presence of premature points of contact, and were the main difficulties found in the patient's carrier of total prostheses. Prosthetic rehabilitation influenced in the patients´ quality life. (shrink)

Introducción: la existencia de pacientes con prótesis totales que presentan problemas para desarrollar sus funciones puede tener diferentes causas, entre ellas el incumplimiento de las indicaciones médicas, y factores capaces de repercutir de forma directa sobre los pacientes, que pueden modificar su calidad de vida en relación con la rehabilitación protésica. Objetivo: Evaluar la influencia del incumplimiento de las indicaciones post rehabilitación protésica en la calidad de vida de los pacientes desdentados totales tratados en las Clínicas Estomatológicas Provincial Docente Ismael (...) Clark Mascaró y La Vigía durante el período de septiembre del 2009 a septiembre del 2013. Material y método: Se realizó un estudio descriptivo en un universo de 254 pacientes desdentados totales rehabilitados. A través de un muestreo probabilístico aleatorio se obtuvo una muestra de 43 pacientes. Resultados: El nivel de incumplimiento bajo se presentó principalmente en el grupo de 50-69 años, con un 50 % y el alto en el grupo de 70 años y más. Los elementos no cumplidos por los pacientes resultaron ser: método incorrecto de sostener las prótesis para realizar su higiene (46,5 %), uso permanente de las prótesis (30,2 %) e incorrecta frecuencia del cepillado de las prótesis (25,6 %). Conclusiones: Los mayores niveles de incumplimiento de las indicaciones de uso, cuidado e higiene de las prótesis totales se presentaron en el grupo de 70 años y más. La calidad de vida de los pacientes con rehabilitación protésica total está influenciada por el nivel de incumplimiento de las indicaciones de uso, cuidado e higiene de las prótesis. Introduction: the existence of patients with total dental prosthesis who present problems to develop their functions may have different causes, among them the unfulfillment of the medical instructions, as well as factors capable of influencing directly on the patients, that can modify their life quality with regards to the prostatic rehabilitation. Objective: to evaluate the influence of the unfulfillment of the prosthetic post-rehabilitation in the life quality of totally umpteenth patients treated at the Teaching Provincial Estomatological Clinics Ismael Clark Mascaró and La Vigía since September 2009 to September 2013. Material and method: it was carried out a descriptive study in a universe of 254 unteeth patients totaly rehabilitated. By means of probabilistic aleatory sampling it was obtained a sample of 43 patients. Results: the lowest level of unfulfillment was in patients between 50 and 69 years old, with a 50 % and the highest one was in patients older than 70 years old. The unfulfilled elements by these patients were : incorrect method of taking out the prothesis for hygiene (46,5 %), permanent use of the dental prothesis (30,2 %) and incorrect frequency for brushing the dental prothesis (25,6 %). Conclusions: the highest levels of unfulfillment of the medical instructions for using, taking care and hygiene of the dental prosthesis were presented in patients older than 70 years old. The life quality of patients with total prosthesic rehabilitation is influenced by the level of unfulfillment of the medical instructions for its use, taking care and hygiene of the prosthesis. Biobanks correspond to different situations: research and technological development, medical diagnosis or therapeutic activities. Their status is not clearly defined. We aimed to investigate human biobanking in Europe, particularly in relation to organisational, economic and ethical issues in various national contexts. Data from a survey in six EU countries were collected as part of a European Research Project examining human and non-human biobanking. A total of 147 institutions concerned with biobanking of human samples and data were investigated by questionnaires and (...) interviews. Most institutions surveyed belong to the public or private non-profit-making sectors, which have a key role in biobanking. This activity is increasing in all countries because few samples are discarded and genetic research is proliferating. Collections vary in size, many being small and only a few very large. Their purpose is often research, or research and healthcare, mostly in the context of disease studies. A specific budget is very rarely allocated to biobanking and costs are not often evaluated. Samples are usually provided free of charge and gifts and exchanges are the common rule. Good practice guidelines are generally followed and quality controls are performed but quality procedures are not always clearly explained. Associated data are usually computerised. Biobankers generally favour centralisation of data rather than of samples. Legal and ethical harmonisation within Europe is considered likely to facilitate international collaboration. We propose a series of recommendations and suggestions arising from the EUROGENBANK project. (shrink)

Participation of the Public in Science: Towards a New Kind of Scientific Practice Participation of the public in science has been the object of an increasing number of social and political philosophical studies, but there is still hardly any epistemological study of the topic. While it has been objected that involvement of the public is a threat to the integrity of science, the apparent indifference of philosophers of science seems to testify to its lack of relevance to conceptions of scientific (...) activity. I argue both that it is not a threat to science and that it is relevant to philosophy of science by showing that it constitutes a new kind of epistemic practice. Two main objections to the idea that the involvement of non-scientists, with their situated perspective and contextual values, can form an epistemic practice will be addressed: the first bears on the epistemic potentialities of the cooperation between scientist and non-scientists; the second on the possibility that this cooperation takes the form of a practice. En este trabajo se analiza la relación que guarda la filosofía del derecho de I. Kant y el pensamiento de K. Marx, sobre todo, en lo concerniente a la «Doctrina del Derecho» elaborada por I. Kant en la Metafísica de las Costumbres . Pretendemos mostrar que la exposición de la «Doctrina del Derecho» lejos de ser el fruto de un pensamiento burgués, como se ha venido considerando desde diversos ámbitos de la tradición marxista, no está tan distante de los problemas (...) centrales abordados por K. Marx en su trabajo sobre El Capital . De este modo localizamos los puntos en común en los que se cruzan ambos pensadores, intentando vislumbrar las posibilidades que esta conjunción filosófica puede aportar en el orden de la filosofía del derecho al presente y futuro de nuestras sociedades. Sobre todo, tratamos de mostrar cómo los problemas abordados en El Capital introducen una suerte de « a priori empírico », que el principio trascendental del derecho debe tener en cuenta para que no sea fagocitado o arrinconado frente al mundo real. Depuis qu'elles existent, les sciences dites exactes se prétendent différentes des autres savoirs. Comment comprendre cette prétention? Faut-il, à la manière des épistémologues anglo-saxons ou de Karl Popper, tenter d'identifier les critères qui la justifient? Peut-on, suivant le modèle nouveau des études sociales des sciences, y voir une simple croyance? Ce livre propose un dépassement fructueux de l'opposition, apparemment irréconciliable, entre ces deux approches des sciences. Et si la tension entre objectivité scientifique et croyance était justement constitutive des sciences, enjeu (...) des pratiques inventées et réinventées par les scientifiques? Réussir à parler des sciences avec humour, sans en faire un objet de vénération, ni de dénonciation, en restant au plus proche de la passion des scientifiques, tel est ici le pari d'Isabelle Stengers. Prenant comme point de départ la critique nietzschéenne du christianisme, cet ouvrage montre que la pertinence de celle-ci est conjoncturelle, et non point structurelle. A partir de concepts élaborés par Paul Ricoeur, la troisième partie articule morale, éthique et théologalité. Loi et désir sont pensés en lien avec la Règle d'or. La revalorisation de l'hétéronomie offre alors une perspective nouvelle pour donner sens à la grâce. Les notions d'attestation-conviction, d'imputabilité, de responsabilité et de reconnaissance permettent ensuite de développer l'identité du sujet éthique chrétien. In critical reaction to the ‘tribunal view’ on experiments, this essay will show the constructive contribution of experimental activity to the processes of modeling and simulating. Based on the analysis of a case in fluid mechanics, it will focus specifically on two aspects. The first is the controversial specification of the conditions in which the data are to be obtained. Although the mapping between form and meaning is often regarded as arbitrary, there are in fact well-known constraints on words which are the result of functional pressures associated with language use and its acquisition. In particular, languages have been shown to encode meaning distinctions in their sound properties, which may be important for language learning. Here, we investigate the relationship between semantic distance and phonological distance in the large-scale structure of the lexicon. We show evidence in 100 languages from a (...) diverse array of language families that more semantically similar word pairs are also more phonologically similar. This suggests that there is an important statistical trend for lexicons to have semantically similar words be phonologically similar as well, possibly for functional reasons associated with language learning. This paper aims 1) to introduce the notion of theoretical story as a resource and source of constraint for the construction and assessment of models of phenomena; 2) to show the relevance of this notion for a better understanding of the role and nature of values in scientific activity. The reflection on the role of values and value judgments in scientific activity should be attentive, I will argue, to the distinction between models and the theoretical story that guides and constrains (...) their construction. The aim of scientific activity is to develop understanding of phenomena, and something that serves this aim and contributes to the development of understanding has a cognitive value. Cognitive values are the features that something that plays a role in scientific activity should have so that it can serve its aim. Las órdenes militares dispusieron desde su fundación de mano de obra esclava en sus tierras, pero la institución que mejor sirve para estudiar la esclavitud en los siglos XII y XIII en los reinos de León y Castilla son los hospitales para la redención de cautivos de la Orden de Santiago. Estas instituciones disponían de espacio para alojar a los cautivos mientras se negociaban rescates con los poderes islámicos, y atendían a la redistribución de aquellos musulmanes que, por no haber (...) sido redimidos, pasaban a ser esclavos de la propia orden o a ser vendidos en los mercados de los reinos cristianos. Los escasos documentos de la sección de Órdenes Militares del Archivo Histórico Nacional dedicados a esclavos ayudan a comprender el funcionamiento del tráfico, trabajo y liberación de este colectivo. Arguments for a just transition are integral to debates about climate change and the drive to create a carbon-neutral economy. There are currently two broad approaches rooted in ethics and justice for framing just energy transitions. The first can be described as internal to the transition and emphasizes the anticipation, assessment, and redressing of harms created by the transition itself and the inclusion in transition governance of groups or communities potentially harmed by its disruptions. In this article, we propose a (...) second approach to ethics and justice in an energy transition, which we describe as systemic or societal in scope. This approach complements attention to the proximate dynamics and impacts of the transition process with a focus on the distant societal and economic outcomes the transition brings into being and how they compare to conditions prior to the transition. It poses the question: do the transformative social, economic, and technological changes wrought by energy systems create more just societies and economies, or do they instead reinforce or recreate long-standing injustices and inequalities? We illustrate this approach with an assessment of one of the most significant existing forms of energy injustice: the energy-poverty nexus. We argue that the energy-poverty nexus reflects configurations of socio-energy systems that create complex, extractive feedbacks between energy insecurity and economic insecurity and, over time, reinforce or exacerbate poverty. We further argue that just energy transitions should work to disentangle these configurations and re-design them so as to create generative rather than extractive feedbacks, thus ending the energy-poverty nexus and creating long-term outcomes that are more just, equitable, and fair.
